School Enrollment Forms

Players wishing to participate in the All-Star Post Season for the 2018-2019 Little League year, must submit a school enrollment form or other verification of residency if home schooled.

LAPS Elementary Students

Please complete the top section of the form that is to be completed by the Parent/Guardian and submit via the link below.

The Little League will coordinate with the LAPS district to complete the remainder of the form.

Home School Players

For those players interested in post season who are home schooled the requirements differ (click here for full details). To express interest in post season play complete the appropriate form, follow the same upload steps, and you will be contacted by Alison Watkins (All-Star Coordinator).
